| Abstract | ||
| To evaluate the significance and advantage of performing percussion tenderness test (PTT) in comparison with rebound tenderness test (RTT) as physical signs in the diagnosis of acute appendicitis. A prospective study carried out in emergency department in Al-Salam General Hospital in Mosul, during the period from January 2002 to 2006. The diagnosis of acute appendicitis is a clinical issue. Percussion tenderness test was performed for all patients with suspected acute appendicitis without taking its result in consideration for the decision of operation and compared with RTT. The validity indicators of PTT and RTT were calculated, which include the sensitivity, specificity, positive and negative predictive values and accuracy, and values were compared with each other. Four hundred seventy patients were diagnosed clinically as having acute appendicitis. These patients underwent surgical operations and the operative findings, in term of the existence of acute appendicitis was made. Out of the 470 patients with clinical diagnosis of acute appendicitis; 418 (88.9%) patients were proved at operation to have acute appendicitis and were considered as the gold standard for classification. The PTT was positive in 380 patients (80.8%), with sensitivity 86.8%, specificity 73.3%, positive predictive value 95.5%, negative predictive value 38.8% and accuracy 84.6%. Percussion tenderness test was positive in 385 patients (81.9%) with sensitivity 88.5%, specificity 71.1%, positive predictive value 96.1%, negative predictive value 43.5% and accuracy 86.5%. Percussion tenderness test is a simple and significant test with high accuracy rate for the confirmation of the diagnosis of acute appendicitis. It is harmless, can be repeated several times during the clinical evaluation and give significant result in the presence of guarding or rigidity. | ||
