| The proper management of fractures of the radial head is difficult and controversial. This study performed to evaluate the long-term results of early and late excision of comminuted fractures of the radial head (Mason type III). During period from December 2005 to May 2008. Forty patients with Mason type III comminuted fractures of the radial head where underwent excision of the radial head at orthopaedic department. Twenty patients had early excision within 1 week and 20 patients had delayed excision within 6-8 weeks. The average age of the patients for early excision was 42 years and for delayed excision was 38 years. All of patients were evaluated for pain, range of movement, strength, stability and were assessed by functional rating index modified from Ann and Morrey through personal interview and clinical examination. In this study 93.8% of patients had excellent results after early excision and 80.38% of patients had good result after delayed excision according to the score system. Postoperatively, 95% of patients had good to excellent result after early excision, and 75% of patients had good to excellent result after delayed excision were not complaining of pain. Also the average loss of movement after early excision was flexion (6o), extension (4o), supination (6o), pronation (5o), and after delayed excision was flexion (22o), extension (6o), supination (22o) and pronation (22o). In addition, we found a mild loss of strength of about 20% after early and delayed excision, and a mild loss of stability of the elbow joint was 5% after early excision and 15% after delayed excision, both had no major effect on the elbow joint function. A measurable proximal migration of radius of more than 1mm was found in 3 patients after early excision and 1 patient after delayed excision but no harmful effect of this was found. The results of study has indicated that excision of Mason type III comminuted fracture of the head radius are better if carried out earlier as far as pain and range of movement than delayed excision | ||
